Medication Error

Medication errors occur when a person is prescribed or administered the wrong medication or the wrong dose of a medication. Medication errors can happen in medical facilities, aged care facilities and in your own home.

Medication errors can happen for a range of reasons including:

  • Mistakes reading medication instructions.
  • Difficulties due to sight issues, language barriers or a lack of ability to understand medication instructions.
  • Being tired, rushing or not paying attention when administering medicines.
  • Unclear record keeping or forgetting if you have taken your medication and taking extra doses.

These errors are preventable through clear record keeping and systems for administration of drugs and ensuring people administering drugs for themselves have a thorough understanding of the instructions for their medicines including doses and times.
